16 July 2011, in the mountainous forest, West Virginia USA. The fly measured 10.8 mm. Overall appearance reminds me of Ptecticus trivittatus but doesn't seem to match that species.

Thanks, Tony, a new family for me. My wife and I went to a wonderful habitat yesterday but somehow this was the only insect I found that I felt like photographing.
